We appreciate and encourage payment for services at the time of your visit, however your request for alternate payment will be respectfully considered. For your convenience we accept Visa and Mastercard. If you have questions regarding your account, please contact us at our office.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Additionally, as a convenience to you, we will bill your insurance company. Most insurance companies will respond within four to six weeks. Prompt payment of your account balance is appreciated. We can make arrangements for a monthly payment plan but that must be done prior to the actual procedure. A finance charge of 1 1/2% per month (annual percentage rate of 18%) is applied to any account over 60 days.
